Physical Examinations

Students registering for Athletics must have a current physical examination on file in the athletic office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the nurse no later than 3 school days prior to the first day of practice/tryouts.

In addition to the online registration, the following forms MUST be downloaded and handed into the Athletic Department as a hard copy (they can be downloaded from the online registration link above):

1) ANNUALLY: Pre-Participation Physical Examination Form (Physicals are valid for 1 year from date of the last exam.)

2) ONCE IN HIGH SCHOOL CAREER: High School Assumption of Risk Form (NOTARIZED) (High School Athletics)

OR

2) ONCE IN MIDDLE SCHOOL CAREER: Middle School Assumption of Risk Form (NOTARIZED) (Middle School Athletics)
